Learning Outcome As a result of this educational activity, I have increased and enhanced my knowledge and competence on: * “Increasing the availability, accessibility and use of evidence-based interventions to prevent or manage chronic disease is a key strategy within the Prevent Chronic Diseases focus area of the New York State Prevention Agenda. The value of these programs and progress toward achieving Prevention Agenda outcomes can be demonstrated through ongoing performance measurement and program evaluation. NYSDOH staff working with the Prevention Agenda has identified a need for local health departments to be able to conduct effective program monitoring of evidence-based chronic disease interventions such as self-management programs as a key component in tracking indicators and goals in the Prevention Agenda.
