“This webinar will provide participants with an overview of the services and resources available from the NYS Office of Mental Health and the NYS Office of Alcoholism and Substance Abuse Services to support discharge planning and the Health Home model of care coordination. It will specifically focus on behavioral health referral and coordination and what resources are available to discharge planners and others involved in long term care coordination in communities across New York State.”
