“This webinar will increase the awareness of the availability of long term care services in New York State for discharge planners and other professionals involved in the discharge planning process. An overview of the changes occurring in long term care will be discussed. The webinar will cover the managed long term care (MLTC) options currently available, who is eligible for the MLTC program, what services are offered in each plan type, and how the services are delivered. Participants will learn about the four managed long term care products and the value of the delivery system. In addition, participants will learn what resources are available to educate individuals and their families upon discharge.”
