Learning Outcome As a result of this educational activity, I have increased and enhanced my knowledge and competence on: * “Until recently, modern public health programs were condition- or disease-focused and funded categorically by either the federal or state government. Beginning with the 21st century, public health began to evolve and address the policies, systems and environments that influence the health of communities, and not just individuals. Health care was transforming during that same period, and in similar ways – working to address commonalities across individual patients, to improve health outcomes. These efforts coalesced with the passage of the Affordable Care Act in 2010, which provided the impetus and the funding to allow health providers to take a more public health approach to their work, leading to the current population health efforts. Assuring access to quality health care is an essential public health function, according to the Department of Health and Human Services Office of Disease Prevention and Health Promotion. As public health agencies partner with health care providers to carry out this function, their historical role as a safety net care provider has transformed to one in which they work to improve the quality of care provided by health care systems. This webinar seeks to ensure all those involved with implementing the NYS Prevention Agenda have a shared understanding of the rationale behind the population health framework being used for those new efforts, and provides a foundational understanding of how this shift across both health care and public health has impacted several NYS public health programs.” 5 - Strongly agree 4 - Agree 3 - Neutral 2 - Disagree 1 - Strongly disagree
