Point of Dispensing (POD) Evaluation Tools
Developed for New York flu vaccination points of dispensing during the 2006-07 flu season.

Free data entry available for New York flu PODs
For the 2006-07 flu season, the Center will provide New York flu PODs with free data entry and analysis of the Staff Feedback/Evaluation Form below. Upon request, the Center will send scannable copies of the form and a return envelope. A basic summary report will be returned within two weeks. Contact us at cphp@uamail.albany.edu to make arrangements. This service is provided under CDC and HRSA grants in order to assist planners in the identification of additional training needs. All data will be confidential and will not be reported to any third party without permission.
Trouble with bubbles?
Some of these forms use a special font to display bubbles next to answers. They will appear in the PDF versions of files, but to see them in the Word versions you need to quickly install the free font “OMR Bubbles” available here.

Note that this is not a complete evaluation kit. It must be supplemented by the POD’s plan, policies, job action sheets, etc., in order to provide evaluators with specific evaluation criteria. The Public Health Emergency Exercise Toolkit is recommended for new evaluation planners.
